First Time Visitors - Please Read

Preparation for your visit is essential in helping us to see you and all our patients in a timely manner. 

Please assist us by completing these tasks before your arrive, if possible.

Please contact your insurance carrier and ask them to make Dr. Raisa Mitelman or Dr. Ravneet Nagi your Primary Care Physician(s).  If you need a provider ID Number please contact our office at 212-420-0104.

In addition to contacting your insurance company, please bring the following with you to your appointment: 

If you need assistance with any of these tasks, our friendly staff can assist you either via the telephone or when you visit our office.  

• Your medical insurance card.
• Any prescription medications you are currently taking.
• A copy of your medical records from your previous Primary Care Physician, if available.
• Please remember that all co-pays are due in cash or credit card only before seeing the doctor.
   

If you are experiencing symptoms such as chest pain, shortness of breath or otherwise feel you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
