If you are uninsured, please do not hesitate to call us. We have a long history of seeing both insured and uninsured patients.

In addition, please bring the following to your first appointment:

  • Your medical insurance card.

  • Any prescription medications you are currently taking.

  • A copy of your medical records from your previous Primary Care Physician, if available.

  • Form of payment. Please remember that all co-pays are due in cash or credit card only before seeing the doctor.
